Job Requirements - Active Registered Nurse (RN) license in Oklahoma (must have license in hand) - Minimum 1 year of recent adult Home Health or Hospice RN experience - Current BLS and CPR certifications (American Heart Association preferred) - Proficiency with HomeCare HomeBase EMR (used within last 6 months or willing to retrain) - Proficiency with OASIS documentation (used within last 6 months or willing to retrain) - Reliable vehicle, valid driver’s license, and auto insurance; willingness to travel up to 75 miles - Ability to complete clinical documentation within 24 hours of patient visits - Open to on-call rotations as needed - Traveler applicants cannot be current direct employees of the facility Primary Responsibilities - Coordinate and deliver skilled nursing care to patients in their homes according to physician orders and agency policies. - Perform comprehensive assessments, admissions, recertifications, routine visits, and care planning (OASIS-driven documentation). - Provide wound care, medication management, pain assessment, patient and family education, and chronic disease management. - Supervise and evaluate Home Health Aides and LPNs/LVNs when applicable. - Ensure timely, accurate documentation using HomeCare HomeBase and OASIS; meet weekly productivity expectations (30 pts/week). - Participate in interdisciplinary team meetings and periodic office meetings to coordinate care. - Maintain infection control practices and adhere to safety protocols during home visits.
